As part of the Welsh Government’s Delivering Transformation Grant 2018-19, ADSS Cymru has completed a report on “Innovative Care Delivery Models in the Community”.
The "Housing, health and social care report" is a specific section within the full 'care delivery models' report, that focuses on research into examples of joint working between housing, health and social care.
The report was commissioned by ADSS Cymru and undertaken by Practice Solutions Ltd.
The report forms part of the publications produced by ADSS Cymru through the Welsh Government Delivering Transformation Grant.
